A personal alarm makes noise. Whether that noise brings help depends on who is close enough to hear it.
This guide covers two kinds of alarm. Keychain sirens are carried and pulled in an emergency. Bed and chair alarms clip to a person at risk of falling and warn a caregiver when they get up.
Neither one calls anybody. So the first question is not how loud the alarm is. It is who will hear it.
- Keychain alarms suit someone who is usually near other people.
- Bed and chair alarms suit a home where a caregiver is present.
- Someone living alone needs a device that places a call instead.
Choosing personal alarms for the elderly
Personal alarms for the elderly fall into two groups, and they solve different problems. A keychain siren draws attention in a parking lot, a hallway or an apartment building. A bed or chair alarm tells a caregiver that a fall-risk person is standing up.
If the person lives alone and nobody would hear a siren, an alarm is the wrong tool. In that case, read our guide to medical alert devices for seniors, which covers devices that call operators or family.
Three carry alarms and two caregiver alarms
Keychain alarms: what the numbers mean
Keychain personal alarms for the elderly are sold on decibels, and the numbers deserve some skepticism. Makers rarely say how far from the speaker they measured. So a 140 dB claim and a 130 dB claim are not a reliable ranking.
What matters more is how the alarm is triggered. Most use a pull pin, which needs a pinch grip and a firm tug. Try that with the person's actual hands before you buy. If grip is weak, a model with a button trigger is the safer choice.
Battery type matters too. Coin-cell alarms run quietly for months, then die without much warning. Rechargeable models need a charging habit instead. Either way, test the alarm once a month, briefly and away from anyone's ear. The CDC notes that a single loud sound near the ear can cause hearing loss.

She's Birdie: simple, loud and easy to stop
130 dB siren and strobe, pull-ring activation
Birdie does one job well. Pull the top ring and a 130 dB siren and flashing strobe start together. Push the pin back in and it stops, so a false alarm in a store is easy to end.
It runs on two CR2032 coin cells, which are included. The ring needs a pinch and pull, so check the grip first. The strobe is useful at night and for anyone with hearing loss nearby.
- Siren and strobe from one motion
- Easy to stop by reinserting the pin
- Very widely reviewed
- Pull ring needs a pinch grip
- Coin cells give little warning before they fade
- Makes noise only, never calls anyone
TECKNET: a button when the pin is too hard
Pull-pin or double-press activation, USB charging
The TECKNET can be triggered two ways: pull the pin, or double-press the button. For arthritic fingers, a press is often easier than a pinch-and-pull.
It charges over USB and warns when the battery runs low. TECKNET quotes up to 90 minutes of siren per charge. However, a rechargeable alarm only works if someone remembers to top it up.
- Button trigger suits weak grip
- Low-battery warning
- Rechargeable, no coin cells to buy
- Needs a charging routine
- Double-press must be practiced
- Noise only, no call for help
KOSIN 6-Pack: one in every place it is needed
Six pull-pin alarms with LED lights
Six alarms cost less than many single ones. That makes it practical to keep one on the keys, one on the walker and one by the bed. Each runs on three LR44 coin cells, and KOSIN claims up to 50 minutes of continuous siren.
The housings are light plastic and the key rings are thin. So treat these as spares and room alarms rather than a lifetime daily carry.
- Lowest cost per alarm
- Enough to cover several rooms and bags
- Built-in LED light
- Thin key rings
- Same pull-pin grip issue
- Decibel claim is not independently verified
Bed and chair alarms for fall-risk seniors
Bed and chair alarms are often called tab alarms. They warn a caregiver when someone at risk of falling starts to get up. A pull-cord alarm clips to the person's clothing. A pressure pad sits under them and alerts when the weight comes off.
They are not a fall-prevention cure. In a cluster randomized hospital trial, raising bed alarm use did not reduce falls or fall injuries. So an alarm only buys a caregiver a few seconds. Someone still has to hear it and get there in time.
Placement of the sound also matters. A loud alarm beside the bed can startle a confused person. A pager-style system sends the alert to the caregiver instead.
Secure MAG-3: the classic pull-cord alarm
Magnetic pull cord for chair, wheelchair or bed
The MAG-3 clips to clothing and tethers to a magnet on the alarm. When the person moves too far, the magnet pulls free and the alarm sounds. A magnet reconnects in any direction, which is easier than threading a pin.
It offers three volume levels and two tones. It deliberately has no on/off switch, so the wearer cannot silence it. However, it alerts only people within earshot.
- Wearer cannot switch it off
- Magnet reconnects easily
- Works on a bed, chair or wheelchair
- Sounds at the bedside, which can startle
- Caregiver must be within earshot
- Clip can be removed by a determined wearer
Lunderg: the alert goes to you, not the bedside
Bed and chair pressure pads with a handheld pager
Lunderg pairs a bed pad and a chair pad with one handheld pager. The alert sounds on the pager you carry, not beside the bed. The pager can ring at several volumes or only vibrate.
Lunderg quotes up to 400 ft of range and about 15 months of life per pad. The bed pad sits at hip level, which the maker says gives earlier warning. Pads are also a recurring purchase, so factor in replacements.
- Quiet at the bedside
- Pager can vibrate instead of ringing
- Covers both bed and chair
- Pads wear out and need replacing
- Caregiver must keep the pager close
- Highest upfront cost here

The table lines up trigger, power and who hears the alarm. Decibel and range figures are the makers' own claims.

Four checks before you buy
- Test the trigger with the real hands
A pin that a strong hand pulls easily can defeat arthritic fingers.
Look forA large ring, or a button trigger.AvoidSmall pins that need a fingernail. - Know who will hear it
An alarm only helps if someone responds.
Look forA plan for who hears it in each room.AvoidRelying on a siren for someone who lives alone. - Choose a battery routine you will keep
Coin cells fade quietly. Rechargeables need charging.
Look forA low-battery warning and a monthly test.AvoidBuying and forgetting. - For tab alarms, think about the sound
Bedside noise can startle a confused person.

Frequently asked questions
Are personal alarms for the elderly the same as medical alert devices?
No. A personal alarm makes noise and a light. A medical alert device places a call to operators or family. Someone who lives alone usually needs the second kind.
How loud should a personal alarm be?
Most good keychain alarms claim 120 to 140 dB. Makers rarely state the measuring distance, so focus on a trigger the person can use rather than a few extra decibels.
Do bed alarms prevent falls?
Not on their own. A hospital trial found that increasing bed alarm use did not reduce falls. They give a caregiver warning, and someone still has to respond.
How often should a personal alarm be tested?
Once a month is a sensible routine. Trigger it briefly, away from anyone’s ear, and replace or recharge the battery if it sounds weak.
Can a senior take a keychain alarm on a plane?
Check your airline and TSA rules before you fly. Coin-cell and small rechargeable alarms are commonly carried, but rules on batteries can change.
For an alarm to carry, start with She’s Birdie, or the TECKNET if gripping a pin is hard. For a fall-risk person at home, use the Secure MAG-3 when the caregiver is close by. Choose the Lunderg pager set when the caregiver sleeps in another room.
Match the alarm to who will hear it
- Test the trigger with the person's own hands.
- Bed and chair alarms warn caregivers; they do not prevent falls.
- Living alone calls for a medical alert device, not a siren.
Personal alarms for the elderly make noise, and noise only helps when someone is near. Pick a carry alarm the person can trigger easily, and a caregiver alarm that reaches the caregiver. If nobody would hear either one, choose a device that calls for help instead.
